  • Government ID for everyone in your household, if available
  • A copy of your lease or rent agreement
  • Recent rent receipt, ledger, or proof of what you owe
  • Recent pay stubs, benefit statement, or income proof
  • ODSP, Ontario Works, CPP, OAS, or other benefit statement if you have one
  • Recent bank statement, if the program asks for it

PHB-SPP was piloted and its households were transitioned into COHB when COHB was expanded in 2020. Included because the bucket named 'Portable Housing Benefit.' Ontario's NHS action plans note a dedicated COHB allocation supporting survivors of gender-based violence; the exact dollar commitment was not re-confirmed on the pages reviewed, so no specific figure is published here.
